So I went to equip Shiva but I can't find it on any person? I haven't sold it. How have I lost it?

Un-equip every materia and then sort them by type and you should be able to find it.

you lose materia if you equipped aerith with it before she died. *OMGZ SPOILERZ*. or if yuffie steals them.
