Pokemon Gold, hands down. I've sank over 2000 hours into that game in the 12 years I've owned the game (I'm still playing the same save file I started when I was 8).
That and Mario Kart 64. Whenever I had my 64, that was my life. Even Pokemon couldn't get me away from Mario Kart.
recently i have enjoyed every bit of DMC, original game, in absolute terms i am really falled in love with REmake
Streets of Rage 2 - I've been playing it since its release and I still get a good co-op play through with a friend of mine every few months.
Burnout 3 - its a great game for a busy life style. Even if I'm short on time I can play a race or two, take out some aggression by running some people off the road and just have plain old fun.
Mirror's Edge - The game is still kind of newish to be on this list, but I've probably played it more than any other game this Gen. What it lacks in storyline I feel it made up for in atmosphere and feel. I think it stands out as unique in a Gen where most of what is out there is a prettier version of an old idea.
Probably Earthbound, but I also really enjoyed playing Halo II mp and I've also put countless hours into Chrono Trigger.
Genre I enjoyed most is Shoot them ups SHUMPS. Since on every system I ever owned/played/or when they had arcades was shoot them ups. I have always enjoyed this genre. Game series Twisted metal series/god of war series or any game series that i had all the installentments. Halo series?
A single game Bubble bobble, Tetris maybe. I've played these lots. One single game I have enjoyed the most? I cannot name a single game that I have enjoyed them most over others. A genre SHUMPS a system SNES a series I cannot since I like many I've played and enjoyed most of them. I just cannot name a single game because I played so many like Mortal Kombat Series over and over, Same with Mario series. I don't have 1 game I enjoyed Ghost n Ghouls as much as Uncharted 2. or Zelda/ Gears of War series.
Tough to pick one game I like enjoed the most in my life.

Basicly its pretty easy to see what my favorite genre is :P so I split it into SP and MP to be fair for the games.
The games that are the most memorable and that i have still been playing untill this day are Croc: Legend of the Gobboes, Spyro the dragon (first one), Super mario bros 3, &super smash bros. Melee. I will enjoy these games untill I die.

Elder Scrolls IV: Oblivion, for me. I spent soooo much time with Morrowind. It was fantastic. Then Oblivion came and it had much better combat, it had fast-travel (a godsend), a much more interesting and exciting main-quest storyline, and the graphics were even more stunning than Morrowind was at the time of it's release. And the large cities were great to explore, shop, train, and steal in.
To this day I consider it the greatest action-RPG ever made, nay, the greatest game ever made. And yes, I have played and own Skyrim. I bought it only days after it released. I thouroughly enjoyed it at as well, but still feel Oblivion is the better game. Especially when you take the Shivering Isles expansion into account.

so many games to choose form... I'll just say my favorite. That would be Majora's Mask. It's not my most played game, but I love it the most over all others.

Final Fantasy 9 and Kingdom Hearts. Both of those games I've lost count with how many times I've beaten them. I also put hundreds of hours into Pokemon Silver way back in the day. Civilization 3 was also a constant attention getter and so addicting.
This is a tough question, but if I had to pick out a few I would say "DOOM", which I have played since it was ShareWare, "Time Pilot" which I started playing in the arcade when it debuted followed by many home versions. "Sky Odyssey" for the PS2 is also a strong contender. If you haven't played that one, you really should.
I really really loved Tekken 3. It's such a special game for me and I l enjoyed practically everything about it. And the soundtrack? Man was that amazing.
Descent II and The Neverhood when I was very young. My first PC game that was really mine was Star Trek Armada II, but then I got Red Alert 2 - that was really special. Still one of my favourite RTS games. Mario 64 blew my mind the way no other game ever has (or will again - only the early Minecraft alpha came close to the sheer wonderment :D. And just starting Morrowind was a little evocative of the magic)
Pokemon Gold was my first non-PC game I owned, and I played 300-400 hours on it. Metroid Fusion and Prime were incredibly influential to me. Phantasy Star II taught me to like JRPGs. And Morrowind made me not like JRPGs anymore xD.
There's a lot. I can't really point at one game and go, DERE. DAT IS TEH ONE I ENJOIED MOAST.
Just for sheer impact, for how much I remember them; Descent II, Mario 64, Wario Land 4, Metroid Fusion and Morrowind are probably the ones. I remember the first Star Trek Armada getting me into RTS too. Damn.

^ Descent II is an awesome pick! I played the heck out of Descent 1 & 2 on PC. The games were networked at my school, so after we'd finish typing lessons for the day, if we had time left in the class we'd spend it blowing the hell out of each other through those cavernous corridors. Goldeneye claims it was first, but we know whatt was really the first really great multiplayer game.
Many of my favs have already been mentioned but I'll still throw them out. Going off of just the sheer number of hours I was willing to put into these games:
Shenmue 1 and 2
Vampire Bloodlines (great idea marred by horrible execution but still compelling enough that I've beaten it like twice)
Fallout 1 and 2
The old SSI "Goldbox" D&D titles like Pools of Radiance (the original, not the remake), Curse of the Azure Bonds, etc.
Ultima 7 plus add-on. Ultima 7 part 2 plus add-on (all time classics and way ahead of their time).
Panzer Dragoon Saga (still need to beat that)
Persona 3 (one of the most unique JRPGs ever. It's funny because I haven't beaten it but I still refer to it as one of the best PS2 games I own).
Privateer 1 and 2
Space Rogue (post-Elite and pre-Privateer open ended space flight/combat sim)
