Jun/Unknown Tekken Tag 2, Heihachi - Tekken, Onslaught - Marvel vs. Capcom, Cyber Akuma - Marvel vs. Streetfighter, Omega Rugal - KOF, Ruby Weapon - FF7, and The End (MGS Snake Eater)
Dark Souls, Four Kings on New Game+. They were a good deal worse than Biggie and Smalls.
The final boss of Gears 3 was a massive pain in the anus.
That ghost bitch in Bioshock Infinite (1999 mode). Terrible, dreadful stuff.
I'm currently working on a 1CC of Progear, and I'm stuck on Volvox (the final boss) and his final phase final spread. I've almost had it ut a dozen times, but I should have it by the end of the month.
The hardest boss I've probably seen though is Zatsuza from DDP DFK or Inbachi from DDP SDOJ. Unlocking the boss(es) in themselves is the most insane thing ever.
all the above are on default difficulty settings. I don't know anyone and can't imagine anyone that would increase the difficulty in any CAVE game.

Diablo III Diablo on the hardest difficulty in inferno mode.
Lots of the Resident Evil bosses... those bastards
I've never been a big fan of boss fights.

I beat it on Z3 and Dragon Arena, Omega Shenron is the hardest
Ornstein and Smough on any New Game playthrough in Dark Souls. Everything about them is absolutely shittastically hard. when I first killed one of them I thought "phew, now I just need to take the other down..." then I saw the cut scene where the other one powers up. I nearly pulled my hair out.
Four Kings is a close second, but I managed to beat them on my second try in my New Game+ run through, but with O and S, I still got my ass handed to me more times than I care to count.
The last boss of Star Ocean: Second Story was so ridiculously hard. He was so fucking fast. I had to use blood armor to beat him.
hm. Indalecio with limiter off. Even with insane leveling he's hard to beat without blood armor. I remember Iselia Queen in that game. She was pretty tough without bloody armor, too, but beatable.
More recently, I'd pick Dominus from Path of Exile. Wow. I mean, you can eventually get your character into a build where you can roll him, but that first time when you're just learning the game and don't know what to expect and/or how to build a proper toon.. Yeah. Ridiculous.
I don’t know if this is only Final Bosses.
But I don’t know if you know “The Ebony Warrior” In Skyrim.

Had to restart soo many times.
I was cruising along until I faced him then it was like hitting a brick wall, I made the mistake of following Prittchard's advice and went to a LIMB clinic to get my biochip replaced and I could hardly see a thing when fighting him so I started a new game without getting that but he's still bloody challenging. I actually brought the turret down the elevator with me which helped a bit.

The Seymour battle on Mt Gagazet. I beat him with Yuna being close to 0 HP. Tidus and Lulu were knocked out and if i didn't beat him with my last attack, i would have died because i had exhausted all options.
It was my first Final Fantasy so i hadn't levelled up appropriately. I am ready to beat him again with styleB-)
Admittedly I haven't played a lot of the FF games that people are listing but the only boss type battle I haven't been able to beat in a video game is the Boss Battles game on Very Hard from SSBB. So close so many times but no dice. Traditional boss battles (unless they are secret end bosses outside the scope of the story) are always possible to defeat if you know what you're doing.
@Netret0120: Yeah that fight was hard for me at first. But I quickly learned an awesome strategy to beat him. The last Seymour battle is the easiest one to me
