Lots of things could be wrong:
r u sittig on the disks
eating them
feeding them to ur dog
ormaybe the disk is just to scratched. Thats what happened to me when i used to have very scratched g-cube didks\
If it's a problem with the disc, then try getting hold of some alcohol-based wipes. They are awesome at cleaning up dirt from inside disc scratches.
Also, i know a lot of game stores now have a disc cleaning service where for a few quid they run it through a magical machine of hope and wonder =]. For a few quid, of course =p

ok if its because of scratches then you can fix it...as long as they arent horrific scratches
1. cover the underside of the disc in toothpaste (colgate is best) and make sure that no parts of the bottom of disc are still exposed
2. leave the disc covered in toothpaste for around 5 minutes
3. after 5 minutes, put the disc under a tap and wipe away the toothpaste and make sure you get rid of all of it
4. dry the disc but make sure not to scratch it
5. try disc out and it should work
it might sound ridiculous but i saw a tutorial for it on youtube and tried it on my broken rayman 2 disc for ps1 and i was surprised to find that it worked perfectly
it was also not coincidental as the game broke every time on the exact same bit, and after trying the tootpaste thing out it didnt break and i continued to play throught the game and had no problems :D
i think that it fixes it by buffering out the larger scratches as toothpaste is slightly abrasive if i recall correctly
but yeah its definitely worth a try. worked for me nyway :D
