Off the top of my head just Super Mario 64 and Super Mario World. I don't find hunting down collectables to be very fun so I almost always don't bother with perfect completions.
For achievements, Dash of Destruction, Borderlands without the DLC and The Bourne Conspiracy. I didn't go around and find all the collectibles in other games before achievements so just them.
Oblivion and Dash of Destruction. I got all of Assassin's Creed 2's achievements but not 100% according to the game. I was missing things like treasures.
The last games I cared about to get 100% were back in the 32-bit era and they were a bunch. The only one that I can clearly remember 100%'ing is MGS: VR Missions. What have I done with most games I beat during this era? sold them, beats leaving them to rot on the shelf.
lol, completed a game 100% as in finishing the game and or maxing out achievements in games.

Ok i've only ever done that with 2 games. Left 4 Dead and Enchanted Arms. Enchanted Arms is very easy because all you have to do is complete the game and you unlock evrey achievment and earn 1000 game score.
