I need an expert gamers advice on finding a game title

Alright, so when I was 6 or 7, I rented this amazing shooter game for the n64 (I think, it might have been the ps1). I really want to go search for it again, yet, I can't remember the title of the game. I can give a description though. You are entering this base with like, a swat team, and you get seperated from your team. In the start menu it shows a list of each team member and either has ALIVE, MISSING or DEAD written next to their name. Some character names are Matt (Black guy who dies at the end of the first mission), Jacob, and Kenny (Who is the only survivor, excluding yourself. Anyone have any idea what this game is called?
