Well, to be honest I've played so many awful games in my time it's hard to single out just one as the worst of them all.
But I guess my choice will have to go to some of those eroge (erotic game) games from the early 80's. Specially those which were made by software houses that became highly prestiged over the years (ENIX for example).
Stuff like Lolita Complex and Mari-chan Ikki Ippatsu weren't just terrible, dreadful games. They were also of the worst taste possible.
Can we make lists or do I have to spend 3 hours to note deciding on only one?
Although Oh Zero One needs to die in a fire:
Predator: Concrete Jungle. No matter how many times I attempt to "play" this game, I just can't do it. Not only was it a missed opportunity, but has a horrible game design that can only be explained as "shallow, simplistic, unoriginal, and boring" which was rather pathetic given its time of release. Weird how it's one of the more moderately expensive titles out there from last gen.
Probably RE: Dead Aim; crappy characters, crappy story, **** controls and gameplay. It was a total piece, and to add insult to injury I paid full price for it when it first came out. . .that was a mistake.
Conan for NES. The game is a monstrosity and is broken in every sense of the word. I'm still angry 20 years later for being pulled in by the awesome artwork and the promise of Conan-style skull crushing.
The graphics are horrible, even by NES standards. And what's sad about that, is that the game was released in 1991, the same year as the Super Nintendo, so it should have been a great looking game since the NES had showed us all it's tricks by then. The enemies spawn randomly, and when I say randomly, I mean anywhere on the screen is fair game, at any time. So you could be in the middle of jump, and and enemy will spawn in front of you, which happens pretty often actualyl, and when it happens you suddenly plummet straight downwards like a falling rock to a fiery death below. And the controls, stiff as a corpse.
Superman 64
yes I did play it, and yes, its a steaming pile.
I honestly understood that it wasn't video games as such that cause violence ,it was simply playing Superman 64 , since after playing it, I too wanted to take my revenge on society, something that was only averted by me playing Street Fighter.
The worst EVER?
I'm going to have to take some time to think about that, I've played so many bad games in my life that it'll be hard to pick just 1.
For the moment I will say Monster Racer on the PS1:
It's a Mario Kart clone but done badly, REALLY badly. I got it for a birthday present from my sister and is up there with the worst presents I've gotten, damn her :x
If I can think of a worse game I'll come back, but for now this will do.
No More Heroes I've been slowly going through this game and it is really hard to even find the motivation to play on. I suppose I'll complete it eventually, but man it sure is far from addictive. Simple game mechanics and stupid mini-games it really boggles my mind that it out scored Wii game like MP3.

"Godzilla Unleashed" on the DS. Even the most ardent G-fan wouldn't find it the slightest bit amusing. It's like the programmer had severe head trauma.

no , though to be honest at the moment I was playing the game ,I was thinking more about going on a rampage through society. maybe Superman 64 was created as an interrogation tool by the CIA.Conan for NES. The same thing happened to me with Rambo. As a little kid, I saw the game sitting on display and expected a game full of shooting, explosions and general badassery. After begging my mother into buying it for me, I was shocked to see that the game actually involved running through jungles and swamps while fighting enemies such as giant insects, killer flamingos and spiders with a knife. What a letdown.
Fairytale Fights (XBox360). Terrible, terrible game that I got for Christmas for some reason. The person who gifted it to me did NO research whatsoever. Normally I would be glad that i got anything, this gift was just plain bad.

I have the 3DO version of it, and its odd, because I can't stand the game, despite the fact Ive heard this one was a 3DO fan favourite.

I heard that as well, and when it was ported I gobbled it right up. Problem was I hated the gameplay from the moment it started...oh well.
I remember finishing that when I was younger, I had close to no PS2 games and my friend borrowed me his copy. I managed to finish the game, but damn, 20 times? You must be a masochist.Shrek 2 or how ever it is spelt. I don't even know why I beated the game 20 times. :| What an awlful movie based game.

Hmmm....I don't think I am... And besides...that was like 10 years ago, about. My friend loved that game for some odd reason. Worst of all, at the time, it was the only game he liked.

I have that game, I haven't played it in over 8 years though, I do remember just walking forward and shooting people though :? Nothing else, I literally don't remember any jumps or anything else than just walking forward.
