These screaming headless dudes from Serious Sam The First Encounter scared me almost every time.

The Long Armed Man in Fatal Frame - the first time you encounter him is when he suddenly rounds a corner when you're in the basement.

The Broken Neck Woman gets second place for scaring the hell out of me though.

Ah yes, Broken Neck Woman scared the beJesus out of me! It was called Project Zero rather than Fatal Frame over here, no idea why.

My vote goes to Pyramid Head too. Not just because he looks freaky and performs unspeakable acts, but also because of what he represents...

I would **** bricks every time they screamed (I was 8. Okay? -_-), so I would try to take them out as quickly as possible. I would throw bombs at every coffin I passed, as a precautionary measure. Good God, those really scared me... oh, and the hand- thingies in the Forest Temple. And the Floormasters.

hmmm..Pyramid Head is scary but I don't think the scariest. Probably Valtiel, then Pyramid head then Twins from Silent Hill 4. They way they point at you from the distance than run up and scream at you. Pretty freaky

Seriously? My mom screams at the top of her lungs when she plays Pac Man because of the ghosts.

Shibito & Yamibito are pretty freaky too, especially since they are immortal..

This was under-rated.

Since you can't fight the monsters and can't outrun them (which most people find out the hard way), every encounter with them is pretty much nerve-wrecking.

While the second picture, which is in fact a baby's head on a spine with what seems to be a mannequin's legs, is not a monster. Its definitely one of the more disturbing things I have seen in a game (it even had me crapping my pants when blood started coming out instead of water)
