Go the the Legacy Forum and you'll get better answers. The oldest game I have played recently is probably Final Fantasy VI though.
Any of the original Splinter Cell games
Star Wars Battlefront 2
Indiana Jones & the Emperors Tomb
Twilight Princess
I picked up Maniac Mansion on the NES from a flea market a few weeks ago; that game came out the same year I was born lol
I'm playing through A Link to the Past and Super Metroid right now. Those are getting pretty damn close to 20 years old.

Amiga Forever:
The Secret of Monkey Island....came out in 1990. Great game.
I was playing the Special Edition that was released a couple of years ago, but I was playing in original mode...so I was basically playing the 1990 version.

Still have the NES hooked up next to the Xbox. I still hop on duck hunt every now and then and the gun and game work as good as ever. Not to mention The Legend of Zelda is my favorite game next to Ocarina of Time. (Yes, my two favorites are both Zelda titles, im a nostalgic loser like that.)
I replayed RE2 last month, played Diablo 2 a few days ago and I just got Planescape Torment yesterday.
PSOne games:
R-Type Delta
Gauntlet Legends
Panzer Front
Mortal Kombat Trilogy
Street Fighter Alpha 3
*Capcom Generations - a 4 disc set which includes:
1942, 1943, 1943 Kai
Ghosts n Goblins, Ghouls n Ghosts, Super Ghouls n Ghosts
Commando, Mercs and another shooter I can't recall
Pirate Ship Higemaru, Exed Eyes, Vulgus, Son Son

I call your Kotor and raise you 'The Legend of Zelda' (first) - it's 25/26 years old. I played it for a couple of hours, but the oldest games I've actually been pouring time into are some SNES classics: Super Metroid, A Link to the Past, Chrono Trigger and Final Fantasy VI -- games I never had the oportunity to play or which I think deserve a re-play.
Yesterday, I was playing the Knuckles portion of Sonic 3 & Knuckles, but I'm also playing M.U.S.H.A, Thunder Force IV, and Galactic Attack these days.
A few months ago i was playing Transport Tycoon. The Yogscast did some vids on it and it look fun. But recently the oldest game ive played is Persona 3: FES. I don't really play old games that much :|
