Definitely EverQuest, back in 1999.
It changed my life in that it was practically my shift from console gaming to PC gaming. It also introduced me to the genre of MMORPGs, and I haven't stopped playing MMOs since then. Since the game was played primarily from a first-person perspective, it indirectly got me into playing FPS games as well, such as Half-Life, UT, and a couple of other goodies.
It also nearly destroyed my life, in that my grades in school pretty much started suffering heavily for a while, because the time I was supposed to spend studying and doing homework was instead spent playing EQ. And while my parents threatened to drop the hammer on me, I recovered my good grades in time before punishment became necessary. Now I maintain a healthy balance of MMOs and studying.
