Getting tackled off a bridge by a horde of angry footballers in Serious Sam 2. Overall the game isn't nearly as good as the original, but the addition of the footballers was IMO a good idea, and it fit in with Serious Sam's weird humour rather well.
Falling into a black hole in Super Mario Galaxy. Heh Heh.
Also, I agree with the flock of chickens one. He's the bloody Hero of Time and he can destroy an evil force bent on killing everything and everyone and he can't even defend himself from chickens? PATHETIC!!!
lol i hate in every free roam game that has swimmiming there are like never places you can get out of the waterxD such as the scarface game other then GTA Sa at least you can like get back on the land fairly quick

for me it was hitman blood money. in the assignment when you have to kill the porn tychoon . if you go one the main house floor and see the red santa girl in the tiht suit. she seduces you and if you follow her into the room she kills you
CS:S on de_dust is home for humiliating deaths. You get pushed by a partner, the thrown grenade gets slighlty offcourse and you frag yourself with a half of own team.
