YesI was playing bad company and got so angry when my vehicle got stuck on a rock I bit the screen on my phone. It broke. Does that count as an accident?

Well, I pressed that on purpose
I was playing Left 4 Dead 2, and my mouse kept jumping around everywhere. Stuttering, skipping, generally being erratic. I kept slamming it against my desk in frustration, until eventually lost my temper and threw it against the wall (it was wireless). Needless to say, it broke quite spectacularly.
I quickly nipped down the shops and paid far too much money for a replacement mouse. Got back, plugged it in, fired up L4D2 again, and I was experiencing the same problem... I then found out I just had to turn off Multicore Rendering in the options menu. I was not impressed.

my worst accident in gaming would be accidentally formatting my console data and deleting ton of long hardworking saves.
my most frequent accident which i get daily would be forgetting/neglecting to save and die.
I turned off my 360 whilst it was saving.. Lost 50% of Kameo
And deleated my F.3 saves after completing the game, completely forgetting about six more bobble heads I needed.. Seem to remember being disgusted with the lame ending having knocked up 200+ hours after sitting through the credits to find no teaser cut scene.
After my first 30 or so hours into Demon's Souls and a lot of leveling up, I accidentally deleted my save. After that, the first time I finised the game I realized that 30 hours are almost nothing on this game, but still, the first hours of the game are the most painful.
A while back when I played Kingdom Hearts II, I was doing all the challenges for the Hades Cup. It was the challenge where you basically had to spam stitch in order to get enough coins or something during the Hades Cup.
Level 48 of 50, I some how paused and exited the Hades Cup in a single motion. What's worse is I think it asks you if you're sure you want to quit.

Also, Playing Phantasy Star Online for the Cube with a friend, accidentally corrupted both save files (Hours upon hours of work) by turning off the cube while it was saving, we never played it again.

Lincoln's Repeater was so awesome....Sometimes in Assassin's Creed when I have to assassinate someone running througha big crowd I accidentally killed several innocents thinking I was going to kill the target, I eventually desyncronized from so many innocent deaths
I picked up a copy of Demon's Souls and started playing. I hadn't heard much about the game, just that it was supposed to be challenging. I was doing just fine and was like, "Pfft! What's the big fuss about this game's difficulty?" I was climbing up some steps after having slayed a demon, but then suddenly my hand twitched, just for a second, and I accidently took my character over the edge. I was just thinking, "Damn...It's okay though, I'll start from my last checkpoint, that shouldn't be too far back." WRONG. I had no idea it took you back to the beginning of the level, you lose all your souls, and the enemies were there once again. I realize it's part of the the point of the game now, but my expression was literally like ":o" at the time. It all happened so fast :lol:
Also, I once saved my sister's game over my file on Twilight Princess, so I had to start all over again -____-
i was playing dmc3 (not special edition) extremely hard difficulty played for 10 hours just going to end the game it was a very hard level and BOOM the electricity was cut also process in dirt
It was in the GOTY version of Fallout 3. I was doing the Mothership Zeta mission and a weird glitch happened that made me unable to progress. I then searched on how to fix it and it turns out that the only way to fix it is by reloading an earlier save. Since I had only one save, I have to start the game all over again :cry:tekken220Wow, you do know you HAVE to play FO3 with more than 1 save, right?.........
This wasn't so much of an accident, more like a bug. But it was pretty unfortunate and accidental on the game's part.
In Nar Shadaa (KoTOR 2) I chose to do it last, so all the other main planets were completed, what's left was the last couple of chapters of the story. Goto was supposed to message me so I could finish the rest of the Nar Shadaa levels, but in the middle of his transmission, a cutscene with Atton suddenly appeared. So I never got the new quest, I didn't know it at the time that that was the gamebreaker. So I saved and played for about 30 more minutes before I realised I could never finish that save.

Around 2000-2001 I finally got good at playing Super Mario 64. I got a file up to 60 or so stars, the farthest I'd ever got in the game. So shortly after that my sister asks if she can play, so I just reply sure and let her play. A few days later I went back on and my file was gone. I went and cornered my sister and asked her why she did that.
Her reply was, "Mario didn't have his hat on." :|
Same thing happened with my Banjo-Tooie file, only it was my younger brother who deleted it.
