I think I died 2 or 3 times, but I have completed it about 10 times on Gamecube, 1 of the "deaths" was when Ashley got shot because I thought I'd be cocky and use the pistol to shoot her in the castle bit where she has to wind the cranks up. One was where I got my head chainsawed off on prupose, the last was where the blind dude absolutely kicked my arse, he kept slashing and I couldn't avoid it...oh well, least I died cool
I have lost count, but I'm pretty sure it's over 30. I'm at chapter 4-1 I think, I'm not sure (what was the chapter in which we were chased by the right hand of Salazar?)
i've died 19 times and i think i'm chapter 3-4 or 4-1 i can't remember. This is also the first i've played through it. I didn't get it for any other console.

I died 30 times, give or take on my first play. Its probably average to die quite abit until you figure out strats, but then sometimes you can't escape the instant decaps and deaths.
I think it was 27 times on normal first time. And it was because of two parts in the game mainly (that caused around 15 kills i guess).
[spoiler] Spoilerish. The two parts were the showdown versus krauser with explosives and the room with the two garradors (blind guys) and ritualists in the castle. [/spoiler]
I only died like 9 times throughout the entire game. And most of them occurred on that stupid run through in the canyon with that huge monster (sorry, don't know what it's called).
Probably a lot (still on my first playthrough); I tend to get stuck on specific areas and die a gazillion times, and then not die for another couple of hours... it's really annoying.
