You can do one of two things:
1. In World 4, find the secret exit of the ghost house and it will lead you to a cannon that will shoot you off to World 7.
2. Beat the boss of the castle in World 5 (the snow area) while you're tiny Mario/Luigi.

How do I save my game progress along the way while playing? I know how to close it to put it in sleep or stanby mode, but how can I save it so that, for instance, if I last closed my DS in the midst of level 1-3, I can next begin from that point, rather than always having to start from 1-1?

Unfortunately, lack of convenient game saving is one of the few downsides to New Super Mario Bros. The only time you can save is when you a) beat a boss (bosses come up 2 or 3 times per world and look like mini castles/towers on the map or b) using coins to knock down the signs that block the entrance to certain areas
Your first chance of being able to save in world one would be after beating the castle/tower level in between leve 1-3 and 1-4. (After beating the level, it will give you the option of saving progress) Or, you could use five coins to knock down the sign on level 1-3. (press up on the control pad and it will ask you if you want to spend five coins to open, then you can save progress)

I am by no means a "hardcore" gamer. Mario may be easier than some games, but I would not call it "easy"...
I just encountered my first "boss" somehow. Mario has to avoid being squished between what look to be concerete blocks. Can anyone offer tips on how to defeat the boss and then proceed further? I killed the guy 3 times and shortly thereafter he reformed... so what's the key/secret?
