You need to have completed every single level as well. Check each world to make sure all the level dots are blue. Especially check the cannons.... You need to use each of them in order to complete them.
after you do that, be sure you've also found every exit. your not done untill you see the message "you have done everything in New Super Mario Bros. Wii" i know that i missed the secret exit in the sandstorm level in world 2

^this, and i think it also includes going to all the mushroom houses which will gain you infinite use of them.

Going into the warp cannon levels (which have no star coins) also counts.
I have been using the gamespot guide, and I have been into all of the cannons. When I go to the peach castle in world 1 for the hint movies, they are all available except for one in world 2,4,6 and 8. Anyone know what these could be? I am pretty sure I have found all the alternate exits.

some of them can be real inconspicuious, like the upper level on the clouds in world 7, or the formaly mentioned secret exit in world two leading to a pipe

I've beaten the game 100% and my stars are sparkley :) I think you get stars for: 1/Beating the game 2/Getting all the star coins 3/Finding all of the secret paths/exits 4/Opening up all the regular paths 5/Using all the cannons/going to all the mushroom houses I could be wrong though :P Also if you beat the game without getting the super guide your stars will sparkle.
