You can only catch it when there are swarms on Route 206. To find out which pokemon are swarming, go to Dawn/Lucas's house and talk to the little sister. She tells you who is swarming.
The swarm changes every day, so you have to wait or adjust the clock to get to the next day and hope that Larvitar is swarming.
It isn't at too high of a level when catching it, so it won't be much of a problem. I reccomend using a high-leveled pokemon that knows False Swipe for catching it (or any pokemon for that matter.)
For your last pokemon on your team, I reccomend getting a flying Pokemon. Staraptor is a very good choice.

Yes, that is the correct house. It is in Sandgem town in the bottom right.
