As for the actual suprise, no one knows for sure. It could have been Brawl online multiplayer, Link's Crossbow Training comes with zapper, or the announcement of Japanese games and new consoles coming to the virtual console.
I think it was the inclusion of the Japan-only games like Super Mario: Lost Levels ans Sin and Punishment on the VC. Though Nintendo never really made any noise about it, so it's just as likely that what was hinted at never really got off the ground.
there were a few minor surprises but nothing huge, the interview where he was talkin about surprises that were cominhe was pressured into sayin an answer and said by the end of september and it blew up into SEPTEMBER SURPRISE ZOMG, it was really people taking something small and making it a HUGE thing and disappointing everyone
