If I'm at home and it's the middle of the night... why bother? Anywhere in public, without a doubt. I'm amazed when people walk straight out of a stall and back into the hallway without even trying to pretend they washed their hands.
I always do. It's kind of sick when other people don't do it. Scoliosis is one the leading causes of not washing after going to the bathroom. That can easily put you in the hospital and if your like me I hate hospitals. :?
Never (at least not in public restrooms), my parts are clean, restroom sinks are not. You get more germs from touching the faucet and paper towel holder.
