I used to be, very frequently. I was the only 11 year old boy with shoulder length hair, hahaha. It became decreasingly frequent as I went through puberty, however, I recently had it once again when an old man walked into the male bathroom where I was washing my hands and asked "Is this the ladies toilets?" :P

Never has happened to me. I remember in the 4th grade not being able to tell if my science partner was a male or a female. She even had one of those either or names I think it was Terry or something.
