Help on form

Sargen Morpho form..

i was born in D.R. but i became a citizen in NJ,usa.  the form i'm trying to fill up says

#10. Place of birth (State for US Citizens - country for all others)

 i think i need to put D.R. here but it's confusing me, i am a US citizen now do they need me to put my place of birth? or just the state i became a citizen?

#11. Country of Citizenship

and here i put New jersey (NJ) but #10 got me confuse.

am i doing it right?

 #10. D.R. #11. NJ
