this one sounds like something straight out of mcgyver.

after a crucial piece of equipment on the space station failed to lock back into place due to debris the crew improvised and used a spare toothbrush someone had packed to dislodge the locking mechanism and then manually reseated the equipment. full story here

I think enough people have died and enough **** has gotten blown up in NASA's various programs to prove that it isn't just about budget and resources. It's also simply an inherently new and risky field, no matter what organization.

They also used a pole and a can of compressed nitrogen. The astronauts Sunita Williams and Akihiko Hoshide are clever.
