I was going to say doctor because of how long they have to go to school, how much they need to know and remember and how they have to react under pressure sometimes but I can see your view as well and add movie star as well because some people just take to it better than others.
I'd say doctor. All the training and then having to put it all into practice, especially when people's lives can be at risk, must be quite a strain methinks.
I think it sort of depends on what you're naturally good at. To one person, being an athlete might come easily and to another it might be virtually impossible.

Both are very broad though. I sing in a band, we're not big by any means but doesn't that make me a musician since I'm using the money from our shows as spending money to get by from time to time? And as for an athlete you can play in a minor league team and be considered a pro. I'm not discrediting either profession but I think other careers are much harder and require a hell of a lot more work.
My main job is a cook at a catering business, I work 14 hours a day between 4 and 5 days a week and I'm nowhere close to being a professional chef. I have my tricks and my own techniques but unless I waste my life at this job I'm never going to make enough to support a family or even myself. Yea its rough making it to the top in the professions listed but its harder on the average man knowing no matter how much he works he can't go out and play a football game every weekend for less that half a year and make millions.
Sports star, it may seem like a dream job but they put so much work into it. Think of the stars on your high school's football team. They're great now but they'll never make it into the nfl.
Politician. Their actions have the greatest consequences out of any job, you have to do your best to keep huge amounts of people satisfied (which is nearly impossible), and its very hard to get into.
Athlete or Politician. Politians have to save face in the public eye, and keep their people relatively happy. Athletes are probably the most popular people in the world, so they need to perform day in and day out, otherwise the public loses interest, they don't get endorsements, and end up that benchwarmer that nobody knows.
