Lots of mentally ill people never even get diagnosed because...there's little basis for forcing a grown-ass adult to even get diagnosed until they're already posing a danger to society. The mental illness thing very likely isn't even going to come out until after they do something f***ed up, and by that point it is a good chance that it's already too late. And besides that, even being diagnosed with a mental illness isn't necessarily enough to warrant locking someone up against their will.

I can't have you arrested because you haven't committed any crime. I can't have you committed. For starters, because my suspicions are not a proper diagnosis. You're not gonna be locked up without a real evaluation by a mental health professional, and there's zero basis for forcing you to undergo such an evaluation. You can't just point the finger at a dude and say, "that guy's crazy, have him evaluated and then lock him up." And even in the event that you do somehow get a proper diagnosis for him and he actually is mentally ill, chances are that that's still not nearly good enough reason to have him committed. Basically, unless you've got evidence that he's planning a crime or is otherwise an immediate danger, there's not really anything that can be done.
