The game is more CPU dependent than GPU... You have a great GPU, but a 3GHz C2D is less than what they recommend (a 2.4GHz Quad Core CPU)... The game eats performance... So you need a meaty system to run it well and C2D isn't the best for running it...
I had an e6750, 8800GTS 640mb combo when I first played GTA4 and it ran like ass... Then I upgraded my 8800 to a GTX 260 and OC'd my e6750 to 3.2GHz and it still ran like ass...
Then I upgraded that e6750 to a q9550 and I can max the game now with no stuttering at all... Just need a LOT of power...

I use the 1.0.4.0 patch and it runs ok on all high except render quality is set to highest. Shadow density is like 16 and I think I have view distance at 50, detail distance at 60 and vehicle density at 34. And the resolution is set to 1680x1050.

8800GT doesn't even have even Vram to play on high texture, so is your screenshot, and you know it too

I just unlock the limiter.
It plays just fine on high textures.
Those screenshots were taken with my 8800GT.
If you don;t want to unlock the limiter then you can just set everything to low and then have only textures at high. Once you are in the game switch textures to low and everything else to high and the texture render to highest and it will not change the textures until you reset the game and all the other settings will be high now.
