In Diablo 2, heroes killed Andarial and Duriel. These are both considered lesser evils. There are four lesser evils total, and two of them were not seen in Diablo 2. Many assume the remaining two lesser evils will be the Act 1 and Act 2 bosses in Diablo 3.
At the end of Lord of Destruction, the worldstone was destroyed by Tyrael. Well the rumors are the worldstone is what largely prevented demons from getting into Sanctuary. With it destroyed, there is now a hole in its place where demons can freely pass into Sanctuary. Current speculation is that Tyrael has been possessed by Diablo and may possibly be the Act 3 boss.
Prime evils cannot be killed. In Diablo 2 they were only banished from Sanctuary. However, with the hole in the remains of Mount Arreat they are free to re-enter the mortal realm. This would mean the Act 4 boss could be one of the prime evils, all three at once, or maybe even some monstrosity where they all combine into one godly demon.
Keep in mind there is really no solid proof of these claims. They are just what people think is possible based on the lore and what's been seen in the limited Blizzcon demos.
