Baldurs Gate 1 and 2, Icewind Dale 1 and 2, Neverwinter Nights 1 and 2, they all have a similar system. Icewind Dale 2 and Neverwinter Nights use almost the exact same rules that KOTOR does, I'm not sure about NWN2 though, haven't played that one.
I'm just talking about the rules though, they're obviously not similar in story or anything.

If youre talking about the whole "dice roll" system found in KOTOR (the random numbers generator that determines how successful an action is), then just about an DnD-type game released has what youre looking for. Neverwinter Nights 1 and 2, Dungeon Seige 1 and 2, and all those other games mentioned have similiar gameplay.
In terms of style, I dont really know of any other games. Thats why I really enjoyed KOTOR; NPCs had dialogue (not just text) which really proved to me the developers went the extra mile, your party is limited to three people including the player, and the perspective was more reminiscent of an adventure/third-person shooter game than a traditional isometric view.
Jade Empires seems to be a lot like KOTOR except set in mythical China or something.
Man I hope they make a KOTOR 3, they better since it is so successful.
Does anyone know if the Deus Ex games (specifically Invisible War, which is easier to find apparently) are similar to KOTOR? I saw a screen shot, and it looked somewhat similar (the dialogue options, similar space setting, money based in credits... )Ice_Hawk_13
Deus Ex (the first one) is a truly remarkable game. I know youre looking for an RPG and not an FPS-RPG (as Deus Ex is), but you should give it a try as it sounds right up your alley. Combat is very much real time and based on reflexes and coordination, but the dialogue and RPG elements will satisfy your RPG hunger. Also, it sounds like you want a more personable game, and they dont get much more personable than Deus Ex.
PS Avoid the Deus Ex 2!
Oh Yes, avoid Deus Ex 2. Deus Ex, is not like KOTOR at all. Lots of people like Deus Ex, I am not one of them.
The closest game I think in feel to Kotor, is Neverwinter Nights 2. They spent a lot of time on character development, and the rule system is basically the same. Actually, Obsidian made both NWN2 and Kotor 2. NWN2 is more linear, but hey nothing could beat having your own spaceship.
Deus Ex is not much like KOTOR. Think more like Morrowind with guns, and you've got it. Although, the dialouge is somewhat similar.getchasome
