Graphics card goes crazy while playing games

Windows XP
Intel Pentium D 3.40GHz

While playing games like Crysis Warhead and TF2 my computer at random goes crazy.

My current installed driver is178.13
newest is178.24 which I'm downloading now.

I'm not sure what else im suppose to update.
Also is there a possibility my computer is getting too hot?
I put my fan speed of the graphic card to 100%.
But I still think something is getting hot.

OH DEAR.............that does not look good at all :| Your card is probably overheating, or the CPU, do something before it dies :o

Well what are your temps? Download real temp for CPU and use Rivatuner for GPU. Is the case dusty? Open the side panel in the mean time.

what is ur PSU ? and how many amps does it have on the 12V rail ? One More thing try cleaning the fans. Gd LUCK !

Make sure all your fans are working and disable any overclocking that you may have set. I have never seen anything like that before.
