NWN2 + MotB or Baldurs Gate 2 if you want to stretch it.
Of course there is also the Witcher:EE & KOTOR.
They're not exactly open world though, but the games are still rich & lengthy

Wow I had completely forgotten about The Witcher, Played it and was a good play through =) I'll be taking a close look at never winter nights 2 with MotB. Oblivion (mods are a must) if you haven't played it. I found it better than Risen and Gothic 3 due to the mods. If you install Quarl's Texture Pack I would say it looks better than those two as well. Also, don't forget to install Francessco's and MMM overhaul mods that adds monsters and static levels to them. www.tesnexus.com has over 10,000 mods to choose from.

Thanks for the criticism, will be taken into context. =)Many of the best ones have been listed (like the Gothic series, Arcanum, etc.), so I'll recommend a few 'different' ones.
You might want to check out Mount and Blade, King's Bounty, Divine Divinity and the Sacred games.

I completely disagree, and would call it one of the best games in the thread (and easily one of the best free-roaming games ever), but to each their own.
