The way you described it... basically every western, fantasy RPG in existence fits the bill. If you're looking for a game exactly like Dark Souls for PC, you won't find any (believe me, I've searched), but here are some RPG games worth mentioning you may like:

-The Elder Scrolls series (especially Morrowind and Oblivion; Skyrim was meh and you probably don't want to delve deep into the past and play Daggerfall)

-Witcher 1 & 2

-Arx Fatalis

-Risen

-Fable: The Lost Chapters

-Dragon Age: Origins

-Titan Quest

-Dark Messiah of Might and Magic

+1 to this list.

Just a correction. Dragon Age Origins is more like Baldur's Gate 1 & 2. It's not so action... like Dark Souls. And Titan Quest is an ARPG. It's great but its more like Diablo, Path of Exile, Torchlight, etc etc...

Kinda. There are different types of swords with different effectiveness on certain types of enemies. There are also other weapons you can use for specific types of encounters - a battleaxe or a hammer for breaking shields, a dagger for causing bleeding etc.

Right off the bat: Dragon's Dogma (... which, of course, is a console-only game mentioned in a PC forum :/).

Also, I can't speak first hand, but a friend of mine who lives and breathes "Souls" games really liked LOTR: War In The North.
