There's not much to it:
1 uninstall your video drivers (either through software icon in your configuration menu; by selecting and deleting the driver in device manager or by using a driver cleaner)
2 power off, open pc and find videocard (the one where your monitor was plugged in)
3 unscrew and unplug (if there's a power connector) and remove videocard from the slot by clipping the little switch (at the back of the slot) and gently pulling the card out
4 insert the new card in slot, insert the power connector cable (again, if it needs one) and put back the same screw to hold the card in position
5 Close your case, and power up. Windows will detect the new videocard and ask you for drivers. Either install the ones on the cd that came with the card or (ignore the windows message and) go to the card's manufacturers website to get the latest ones. You will need to reboot after installation, after that you're good to go.
