it has to the with the timing.

your first click orders him to attack. if you click the second time too fast, it'll be "too fast" you have to wait until your cursor turns flaming to hit the second time

It's all about the rhythm. Click once, wait for the cursor to change colour. If you click when it's red, you attack more vigorously. If you click when it's yellow, you attack less so. And if you click before or after the colour comes up, you won't attack at all. It's the developer's way of changing up combat; there's no click-fest here. It's been a while since I've played it, but as you level up your skills with a sword and depending on which stance you choose to fight in, the timing patterns come at different intervals.

ya, the combat is fairly clunky at first but when you get a hang of it its fun to watch.

When you get your sword out, you usually have to wait for the "unsheathing" animation (when Geralt pull the sword out and gets it ready) to stop, then click on an enemy. Depending on how far away the enemy is, Geralt will either swing the sword or will run up and then swing it.

ONLY CLICK ONCE AT FIRST! The combat is based on combinations, and it is not a clickfest. So click once, then wait for an orange (high damage) or yellow (medium damage) mouse cursor to appear then click again before this disappears. I think you can chain 3-4 combos this way.
