5 free Blu-Ray HD movies?

I'm having some problems with this order form.

It asks for my UPC code? Well no idea which code I'm supposed to give and where to give it. Like do I just write it down on a piece of paper?

I don't want to send this form in and get a reply saying that I forgot something or misunderstood something.

The UPC is the barcode area on the box itself. Use scissors to cut out the whole white barcode area.

There are two parts, one with a single bar code and another with three.

The UPC the one they scan for purchase.... also I wish they had better movies to show off blu-ray. the movies quality are average while there are a lot that look muchbetter than the options for the free five.

Casino Royale is a good example of what blu-ray can do, also pirates.

It should be the one with only one barcode and should say POS(Proof of Sales) UPC above the barcode just cut that one out and send it along. Then be prepared to wait awhile to get your movies

I like how the form doesn't even mention actually cutting out the code.

Well I got my code, receipt, and a filled out form now.
