If they are going to play online people sometimes swear. And there is blood to. Its rated M so it depends what age your children are. If they are under 12 or 13 i would not really reccommend it.

One thing to cut down on the swearing in the online is to pair a headset with the system and then just put it on the shelf or something. Then they won't hear the talk through the headset just the game itself.
people do swear a bit online but usually not too much and yes there is blood but not too much, mostly the reason cod4 is rated M is because of how intense the combat gets
there is one bit where you see a guy get shot in the head real close (but that does not look that bad). But other than that there is not much swearing or anything. But obviously there is some blood.
