He's Solid Snake. A clone of Big Boss. His body is breaking down cause he's a clone (that's why he's old due to rapid aging)

He has the eyepatch because it's some sort of technology-ma-thingy. He still has an eye under there. Anyways, as for Big Boss, it's assumed that he was destroyed for good in Metal Gear 2 (NES). I think.
