one controller for all the guitar strory modes, you can also use the guitarhero2 controller (on the 360, not sure on the ps3, but since its USB it should work)
I honestly don't think there will be separate guitars. I mean, they could do that, but why would they? In GH2 you could play bass with a PS2 guitar controller. I don't see there being a bass guitar specific controller. Maybe the controller style would be bass, but it certainly shouldn't be a gameplay mechanic.
I'd assume there are going to be bundle packs, one of them including two guitars, a microphone and drum kit.
Rock Band will use the same controller for both the bass and the guitar. As to your second question I am a little confused. If you are asking if you can play the Bass line and the Guitar line as one player then the answer is no.
