Deadhand. Nothing left such a strong impression on me (impression read scars). And ReDeads. And Floormasters, not really scary, just made me jump off my chair ever gosh-darned time. Pretty much all of Shadow Temple as well.

This guy chasing me in Mario 2 used to scare the crap out of me when I was like 5 years old and sent chills up my spine. I think it was his relentlessness in pursuing you.
