I think Infinity Ward has most improved. CoD4 surpassed anything they had done before.
I'm disappointed in Blizzard. They've published nothing but WoW in the past 5 years.

QFT. Company of Heroes was a revolution, and Dark Crusade was a hundred times better than Dawn of War, but AoE3 was a major disappointment.

Certainly, Relic has produced some fantastic games in the last 5 years, but I wouldn't say they've improved that greatly over HW, because let's not forget, HW was fantastic too.
As for Ensemble, I agree. They went from making the best RTS ever (AOEII), to making a merely acceptable sequel (AOE III).

I was addicted to AoE2/AOM a couple of years ago, but now I find historical RTS games kind of... boring. I would have liked CoH even more if it was set in Modern/Sci-Fi.
Nintendo and Blizzard - It's hard to improve upon perfection;)Popadophalis
Nintendo and Blizzard have been consistently putting out high quality games for many years so it's hard to say they've really improved. But then, similar to what you said, once reach the top where do you go from there?
Most improved-Retro Studios, from a ton of unnanounced games to the MP series.
Least improved-Rare (they've made some games but they've dissapointed on a lot of others.) and EA (They used to be ok, now they make about 1 good game a year, sometimes 2.)
Valve has improved so much since Half Life 2. They've been doing a lot more with their Source engine like remaking their old games like Counter Strike and Day of Defeat and making new IPs like Portal. Valve's Steam service has been doing great over the past year or so. Valve's new and improved.
EA games are getting better.
Least improved would be Factor 5. Its all been downhill since Rogue Squadron.
Least improved: Ubisoft.
Besides totally screwing up any Wii game they release, they also release only crap on the 360 and PS3, and they ALWAYS go for the "easier = better" approach. Just wait. The next action game they release will use only one button and require you to run only in a straight line.
They're even worse than EA at milking their franchises. At least with EA, you KNOW which of its franchises it whores out, and EA keeps those franchises separate from their REAL ones like Crysis and Spore. With Ubisoft, they whore out EVERYTHING they make, but pretend like they're still relevant in game innovation.
Most improved - EA. Even though they're still evil and suck they've actually started making some good games and not just annual rehashed trash.
Least improved - Last 10 years I'd say Nintendo. Been a downhill slide since N64 days imo. Last 5 probably Blizzard since they haven't released anything but WoW. We'll see with Starcraft2 and whatever else they've got going in secret.

Most improved: Atlus - 5 years ago it was a selective audience cult developer, now it's trendy and popular.
Least improved: Nintendo - Same stuff as always.
Most Improved: Bungie Studios - Halo is one of the most renowned franchises out there, and it's regarded as one of the best FPS franchises of all time.
And I would have to agree with Rare being the least.

Valve's done nothing new since Half-Life 2 except Portal, which has two hours of gameplay. HL2 episodes are merely "more of the same", and I dare say that episode one was worse than the original.

The only ones that left are the ones who made Goldeneye and Perfect Dark.
Banjo 3 will bring Rare back to the top of the gaming world. Quote me if you like.
