2nd Death: He was blown up by Frieza.

3rd Death: He was eaten after being turned into candy by Majin Buu

4th Death: He was killed by Super 17

and if you count his Trunks' future timeline counterpart.

5th Death: He was murdered by Androids #18 and #17

It's been a while since I've watched DB.. But taking into consideration that when Goku fought Gohan at Baba's tournament. Gohan reminded Goku that his tail was a weakness, this leads me to believe that, Gohan would've been the first person to grab it. So my answear is Gohan (Not his son, Gohan)supergohan1
