Did you see the emotion Cena showed after Randy Orton kicked his father a year or so ago? That was definitely real, Cena can't act like that. The fact that you'd think it's fake is kinda insulting.

Soap Operas have much better writing.Personally, I hate wrestling being known as "fake". I prefer the term scripted. The reason being the wrestlers are putting their bodies on the line while they wrestle, that's not fake. They all just follow a script.-Static-

Many of the bumps are real, with exaggerated reactions thrown in for drama emphasis. I mean, there is some pain, but often not much as it may seem to you. The punching/kicking is often even more fake. You have to look no further than Boxing to see that is the case.
Now, there are cases when pain is real. When things go wrong. When accidents happen. That is when injury happens. And often, when a guy is out on injury in the story, the injury itself is true. Now, whether or not the story the WWE tells you how it came to be is true depends on circumstances. Often, however, the how is what is fake. For example, about a year ago, while John Cena really was forced off WWE television for a couple of months due to a real-life injury, it happen due to Cena incorrectly delivering a hip toss to Mr. Kennedy, not because of the post-match stuff with Randy. In other cases, the injury is story only. Good example would be Batista-Michaels from last year. Michaels, apparently, had some sort of leg injury. Jericho claimed that Michaels was lying. That he was fine. Later on, it was found that Jericho was right, with Michales, down the road, confessing to this, not before using this to his advantage to win match against Batista at Backlash 08.
When someones bleeds, often, the blood is real. However, in general, not cause from a chair-shot. That is just for purposes of the story. How the delusion happens is that when the wrestlers is down on the mat, he uses a small blade, that is mostly tapped up, to cut himself across the forehead. Usually, the wrestler will have this blade hidden somewhere on this body, or the referee with have it hidden, and when comes time, give it to the wrestler. Usually, everything goes as plan. However, things can go wrong, and on rare occasion, someone bleeds more than normal. Probably the most infamous example of this would be the JBL-Eddie match from Judgement Day 2004. Eddie bleed so much, he had to be rushed to the hospital afterwards. Also, notice I mention forehead? That is because when they bleed from the forehead, chances are, it is from this method. However, mistakes happen, and usually, when they bleed from an area on their body other than their forehead, chances are, it is real.
Otherwise, from the stories to the outcomes, all fake. In some cases, such as Matt-Edge feud from a few years back, has some basis in real-life, however, do not let that confuse you. Fake, nonetheless. General policy is that if it is on television, it is fake.
About the only time when that policy was not true is with the Montreal Screw Job. To cut the story short, Bret Hart thought his match with Shawn Michaels would end in a DQ, with Bret dropping the WWE title the next night. Up until Shawn Michaels begin to apply the sharpshooter to Bret Hart, Bret thought everything was going to plan, with the keyword being "thought." This turned out not to be the case. Much to the surprise of Bret, and most everyone watching in the audience and at home, ref called for the bell, the match was over, and Shawn was the new champion. On screen, it looked as if Shawn was upset over this, when, in reality, he was in on it. Now, granted, there are conspiracy theories out there that say Bret Hart really was in on the screw job, however, the general belief is that Montreal Screw Job is legit.

It surely is pre-determined that is for sure and the story is all scripted. Most strike are usually not fist strike like they are seem to be. If the person getting hit with a move look real, they are "selling" it well. Botches occurs from time to time that may actually injure which are of course real. I believe that some of the blood are real by having some them cut themselves with a small razor while other just simply use capsules. Do anybody really think that any human can really stand up after being nailed in the head with a sledgehammer? Probably already explained with the long post above.
