can't get a data migration kit?

So i recently found out that microsoft will send you a free data migration kit if you fill out the form. However all the links to the form don't work, does anyone know if micorsoft has stopped supplying free migration kits?

My problem is I live in North America and I don't know where to mail it to. They have an "If you live in Europe" section on the PDF itself, but no address for North America. The only thing I found said to send it to their fulfillment center. I have no idea where that is.

i think you can do this over the phone as well. just provide information and i think the number on your new hard drive. they should be able to send it to you without u going doing the paper work. not 100% sure though, but i would give them a call if i were u
