Every game, I never get the call about Jo or the Sean, and I can never fight them. I' ve looked all over and everything I try doesn't work.

Either you're in the area where the call is supposed to send you or you have too many scoops up. If either of those happen you will not get a call from Otis.

I think Matt Rorie made a game guide for this, and it shows the exact time a place you can fight the psychos. Go check it out.
