i just beat cod4 on veteren and it didn't give me the deep and hard achievement yet i got the "won the war"achievement. Have any clue why? (and yes i checked the mission select screen....they all say Difficulty Completed:Veteren)
After the game ends, you have to sit through the credits and there is a bonus mission where the SAS are running through an airplane to rescue a hostage... you have to sit through the entire credit sequence to unlock it... there is an achievement for beating it on any difficulty and another one for beating it in less than 1 minute on Veteran...(and that is hard)

I got that achievement before the mile high club. mile high club is not part of the campaign.

Oh yeah... Deep and Hard is beat on Veteran or Hardened... so go on the mission select screen and look at the "Highest Difficulty Completed" section of the information... Make sure you have them all beat because I have never heard of this one glitched... if you do have them all completed, call support of Activision and ask them what to do...

yea i unlocked the extra mission on the plane and i skipped the credits and yes all of the missions say i completed it on veteren...im angry >_
Did you beat any of the game with cheats enabled? Cheats disable achievements from unlocking. It might be you beat a level with cheats active? I know they disqualify you from the intel achievements not sure if they do the same for the rest of the achievements, I imagine they would.
