No they do NOT stack. I beat it on Pro and had to beat it on Expert and Normal afterwards to get the achievement for that. And you do actually have to get 5 SAs in each difficulty. After you gain a familiarity with all the levels, it's easy.

EDIT: Dude, I looked at your Gamercard and it says you only have 75/1000 for Hitman. Why comment if you really don't know?

There are 4 difficulty levels. So, you have to play the game once for each level to get all the achievement points. I have all 1,000 points for Hitman. :)
