First I was programmed by fontaine correct? So I never lived on the surface just my life started when? During the burning plane?
Why did Ryan ask me to kill him with the golf club?

OKAY.
You were born in Rapture, and you were placed under Fontaine's mind control, and all your memories are pre-fabricated. Actually, I don't know if you were born there or what, or if you weren't put under control until you were older, not too sure. The GAME starts off on the plane, but it is implied the character is going somewhere and that he has a past.
As for Ryan... not sure. I guess he knew Fontaine would take him out or something, or he was just a sadistic man.
Also, I guess you get that ending by saving little sisters. I don't know if you have to save all of them or just a majority. What did you do?
Sounds like you got it all right man, at least from the way I envisioned it. Your life before the plane crash was almost prefabricated, and then the package was like a message, almost like in "The Manchurian Candidate" and then you were the one who caused the plane to crash basically right on top of Rapture. Now, Ryan asked you to kill him to prove a point. He was your father, in case you missed that, and throughout the whole killing him scene he was saying "A man chooses, a slave obeys" so he was showing you that you were a slave to Fontaine. Also, just a little speculation here, but did you see Ryan's body when you ran back through the room with the Little Sister? No, well I didn't either, remember the Vita-Chambers were genetically coded to work only with him (and you obviously since you are his son), so perhaps Andrew Ryan knew he wasn't really going to die....or maybe I'm just crazy.
But anyways, I hope you had fun with the game, as I thought it was the best piece of art/storytelling I have ever played. Peace dude.
