Battletoads on NES, but I assume your looking for hardest 360 game COD4 on veteran. I'm still in the process of doing it (course I just started this month)
As mentioned, cod 4 on veteran was a little bit of a **** still a couple of levels I can't do. Some of the gow 2 on rails bits actually seem impossible on insane, but mabye that's my fault for having no friends to play them with. The brass balls achievement on Bioshock can be tricky also.

Ninja Gaiden for sure, I can't tell you how many times I thought about just breaking the disc, couldn't though because it was a rental. But still a good game once you get the hang of it. -_-
Personally COD 4 was easy on veteran compare to COD 2 on veteran especially that mission i think its called Hill 400 or something but your in the open were enemy's just keep coming and coming still beat It but it was hard.

Yea...Cod:waw was the hardest/cheapest game i have ever played, Cod 2 was a little tough but Cod4 was very easy...well at least for me. COD4 very easy? on veteran? If you say yes then im gonna kick your *** in my imagination...lol... I've found that your "team/squad" is totally useless on veteran. Yeah contras are hard. Megaman 9 was hard and seemed very cheap to me. I've never had any trouble with 1-8 + the few Xs I played. beat X4 and X5. never did bother beating sigma in X6. Ultima Exodus and Marble Madness on NES
Ninja Gaiden Black: If it wasn't for the NINJA DOG easiest difficulty setting, i would never have won.
Dead or Alive 4: Anytime you can get stuck on a character in a fighting game for 45 minutes (as with the cybernetic kasumi who can kill anyone with one combo), even on the hard difficulty, the game stops being challanging and just annoying.
 COD games on veteran aren't hard per-say, just cheap. I can't count how many times i was playing HEAT on cod4 only to be struck by a stray bullet right before i got to the landing zone. it seems unfair to double the enemies and half your health. smarter enemies would be a better way to make the game harder. Â

The original TMNT for NES and Battletoads for Sega were hard. For 360 I think the hardest I have played is Ninja Gaiden; I don't think any of the CODs were that hard (have not played W@W yet).
I would have to say Animal Crossing. That game is unbelievebly difficult. Made me wanna punch myself in the face.

So I take it you've completely beaten Animal Crossing? It's not hard, just tedious. Anyways, I say CoD4 on Veteran, especially No Fighting In The War Room.

uh huh, or you could have just said it yourself rather than waiting for somebody else... I never played 2. I started at 3 and meant to play the first two but never got around to it. Never played the new ninja gaidens but the NES ones were ridiculous

Ya dude that game was sick, the scooter level where you have to dodge the walls killed me every time. I don't know one person who's beaten that game legitimately (as in not a rom with save states and what not). for me and my bro it was another level like the scooters only you were in little airplanes. maybe 4 levels after the scooters. The running man on the Spectrum ZX i didn't get past that first dog. you could punch but you couldn't crouch and punch so you could not hit the dog. it was a about 4 seconds into the game. Team Ninja are bastards. Path of the master ninja was ridiculous
Ninja Gaiden 1, Ninja Gaiden 2 was way too easy on my first playthrough and that wasn't on the easiest difficulty. For me it's close to impossible on hard though 0_o
Man you guys think Ninja Gaiden 2 is hard, try playing Ninja Gaiden 3 on NES. I was obsessive when I was a kid. I woudn't rest until I beat a game. I beat 1 and 2, but actually gave up on 3. I did beat it many moons later on my PC but only because I could save the game at any point. Current Gen games don't compare to NES because back in the day games would set you back crazy far after you died.
Recently it would be Ninja Gaiden 2 on Master Ninja. All-time would probably be an old school quarter-muncher like Gun.Smoke or Robotron, the types of games that were built to kill you quickly and take your quarters
