Since I didn't include this in an earlier post, I'm putting it up now.
The one that comes to mind for me and probably many Xbox gamers was Shivering Isles for Elder Scrolls: Oblivion. It was like a totally new game and the art was incredible.

For me it was shivering isles as well. It was the most memorable part of oblivion for me. The city of crucible was my favorite locale. I don't know why, but I found its run down buildings, sludge filled streets, and crazy looking city gates quite charming. Speaking of which a friend of mine has a theory that the sheogorath in skyrim is in actuality the hero of kvatch. Don't know about that ,but since sheogorath does leave you his domain in the shivering isles it could happen.
Shivering Isles.
I loved the visuals, and Sheogorath is one of my favorite NPC's in ANY game to date.

As several others have already said, Point Lookout for Fallout 3. The atmosphere, new enemies, new quests/story, and new items were all just so awesome.
