for me, it's a tie: Wasteland Survival Guide, Skorge boss fight on insane & Raam boss battle on insane.

The end level to one of the episodes in N+, I can't remember what number it is.

N+ is really, really annoying.

Halo 3 legendary and all the Skulls.

Off the top of my head, probably F.E.A.R on Extreme. I died quite a few times before I cleared that one.

The Pointe Du Hoc mission in Call of Duty 2 on Veteran was quite ridiculous. I'm fairly sure it would have been easier to do it in real life.
