Prince of Persia Classic's Survivor achievement - beating the game without dying. Sure, the game only takes 20 minutes to rush through when you hurry, but not dying in that game is ridiculously hard even when you're being careful.
Well out of the games you've listed, Fable 2 is the only one I've played (on the 360 at least, I already played bully on the ps2). I remember when I beat gears of war on the hardest difficulty, I was pretty proud of that.
Flushed from The Orange Box. Killing someone with a toilet is priceless. Also Master of Disguise from The Orange Box. Once that enemy medic heals you you can backstab then taunt the hell out of them.
Big Cheese of the South Seas in Hexic HD. I only have the grand pearl pooh-bah left to get but when I got that achievement I had a few choice words left by buddies in my xbox in-box :-)
Mega Man 9 - Gamer's Day (beat the game 5x in one day).... i'm currently trying to end the game without energy/mystery tanks and ending the game without dying...
