I have an Xbox One and about 3 times a week ether a game will crash mid game or my Xbox One will go to a Black screen when I hit the Home button. I have seen a lot of things about the Xbox going black screen, but no one has figured it out yet. I know how to fix it, but is there any way to stop it. On the other hand, I have not seen may threads about games crashing mid game. Ether I will be playing Ghost or my girlfriend will be playing The Lego Movie Video game and mid game it just take us to the home screen and we have to start the game all over again. I'm also having a problem with my controller disconnecting for about 5 seconds every now and then. Not really a big deal unless your in the heat of the moment in Ghost and that 5 seconds of disconnections gets you downed in Extinction or you lose you 15 kill streak in multiplayer. I just wanted to put this out there, but most likely this forum has already been posted a 100 times and someone is going to yell at me to use the search function before posting it again, but o well. I hope someone will give me some useful information and not flame me please. Thank you.

Sorry to hear about this my friend, unfortunately I'm not sure what the issue is you're experiencing, I myself have not experienced anything like that before. This might not be the answer you want to hear but give Microsoft a ring, or you can use their online chat method which is 24/7. You've got a 1 year warranty with your console, don't be afraid to use it.

I've had this happen to me also but not that often, sounds like something is up, maybe a faulty hdd or something. I'd just return it and get another one.

Go into your settings and turn resume games fast off, my xbox was doing the same thing until I turned it off. It has worked without issues since.

Had it happen to me once. All you have to do is unplug the console for about 5 minutes, plug it back in and everything is back to normal. Some sort of issue with RAM or something I'd assume. Hasn't happened since then. I disabled resume games for awhile because I read it might have something to do with it, but have since reenabled it and haven't had the issue again.

@charles456k: Xbox One has launch issues like every console does. It will take months for Microsoft to get it running at its best. I wouldn't be surprised if it doesn't get fixed by the end of the year.

What DarthGumballs said is 100% accurate, it is a RAM issue. Has happened to me once, and the other option is when turning off the XB1 hold the power button on the console down until it turns off. This is a hard reset (does not affect your hard drive or anything, it more similar to shutting down and powering up again instead of closing the screen and putting it on standby). Unplug the power brick for 30 seconds to 2 minutes and it will be working fine again.
