Also, how is the word offensive? Inappropriate, perhaps, but I don't get the offensive part.

My personal opinion is that people are going to think offensive things, if it comes out of their mouth, at least you know what they're thinking. I've been called all sorts of nasty things in life and sometimes you just need to learn to accept that people can be mean and get over it.

Zero tolerance is sort of a tricky thing, because sometimes it's hard to say if a comment had malicious intent, and if they go about giving really harsh punishments when people say something that they didn't mean to be intended as racist/etc., that might not be good for the site. It's hard to say.

The Top Games list goes strictly by rating and release date, which I think makes for a list that is not very useful to the average user. LittleBigPlanet got 9/10 and could very well be on that list, but some games that instead made it on are expansion packs, XBLA games and new ports for old games (Mass Effect). What we're ending up with is sort of a mish-mash of well rated stuff that doesn't necessarily point out which games people should try. Some people might want to click on Top Games and see all standalone games that aren't XBLA or a year-old game that was ported months ago, like LittleBigPlanet, Far Cry 2, Dead Space, Fable II, etc., and I think adding an option that allows people to view more recent, more relevant games would be a great addition to the site.
